ABSTRACT

Objective To investigate the preventive therapeutic effect and possible mechanism of single chain variable fragments chimeric protein (SD) of ovalbumin epitopes internalizing receptor DEC-205 antibody on food allergy in mice. Methods Mice were randomly divided to five groups (control, PBS, scFv DEC 100 μg, SD 50 μg, SD 100 μg) and treated for 24 hours before OVA administration. After challenge, the serum level of OVA-specific IgE, IgG1, IgG2a and IL-4 were detected by ELISA. Infiltration of eosinophils and mast cells in the jejunum was observed by HE staining and toluidine blue staining respectively. The bone marrow of tibia and femur was isolated and cultured to obtain immature dendritic cells(BMDCs), which were further treated with LPS (10 ng/mL), TSLP (50 ng/mL), scFv DEC protein (1000 ng/mL) and SD protein (10,100,1000)ng/mL for 24 hours, and the IL-10 level of supernatant was assayed by ELISA. Results Compared with PBS group, the number of SD-treated mice with diarrhea was markedly reduced. The difference in rectal temperature and the levels of serum OVA-specific IgE, IgG1, IgG2a and IL-4 decreased significantly after prophylactic administration of SD; The number of eosinophils and mast cells in jejunum also decreased significantly while the IL-10 level in the supernatant of BMDCs increased significantly after SD intervention. Conclusion SD mitigates experimental FA response by fosters the immune tolerance property of dendritic cells.

ABSTRACT

Objective To explore the expression of peripheral T cell subgroup (CD3+,CD4+,CD8+,CD4+CD25 high regulatory T cells) and the level and significance of serum cytokines in patients with silicosis.Methods One hundred and six cases patients with silicosis were collected in the Fifth People''s Hospital of Suzhou as study subjects and 56 healthy subjects as control group.Flow cytometry was used to detect the peripheral CD3+,CD4+,CD8+ and CD4+CD25 high regulatory T cells (Treg) of the patients and the control group,while chemiluminescence immunoassay was utilized to measure the peripheral serum soluble interleukin-2 receptor (sIL-2R),interleukin 6 (IL-6),interleukin 8 (IL-8) and tumor necrosis factor α (TNF-α).Results (1) The percentages of peripheral CD3+,CD4+ and CD8+ in the silicosis group were all lower than those in the control group (t=3.755,3.828,2.347,P<0.05);the percentage of Treg cells was higher in the silicosis group than in the control group,the difference was statistically significant (t=-8.345,P<0.05).Compared with the control group,based on the one-way analysis of variance,the differences in CD3+,CD4+,CD8+ and CD4+CD25 high cells were all statistically significant (F=5.620,8.007,26.71,P<0.05);in the silicosis group,the percentage of CD4+ T cells was lower in stage III than in stage I (t=3.424,P<0.05);compared with the control group,the percentages of Treg cells in the silicosis group were lower in all stages (t=-7.934,-9.445,-5.096,P<0.05).(2) The levels of peripheral sIL-2R,IL-6,IL-8 and TNF-α in the silicosis group were higher than those in the control group,the difference were statistically significant(t=-6.952,-4.506,-2.551,-5.670,P<0.05);compared with the control group,based on the one-way analysis of variance,the differences in sIL-2R,IL-6 and TNF-α in all stages were statistically significant (F=11.03,11.31,13.22,P<0.0001);the sIL-2R was higher in patients with stage III silicosis than that of stage I (t=-2.882,P<0.05);IL-6 was significantly higher in stage II and III silicosis group than that of stage I group (t=-3.022,-2.632,P<0.05),and TNF-α was higher in patients with stage II silicosis than patients with stage I silicosis (t=-2.322,P<0.05).(3) The level of peripheral Treg cells was negatively correlated with the percentages of CD3+ and CD8+ cells in patients with silicosis (r=-0.357,-0.508,P<0.05);sIL-R2 was positively correlated with IL-6,IL-8 and TNF-α,respectively (r=0.483,0.199,0.392,P<0.05);TNF-α was positively correlated with IL-6 and IL-8,respectively (r=0.338,0.338,P<0.05).Conclusion Patients with silicosis have abnormal expression in peripheral T cell subgroups,significantly increased Treg cell and dysfunctional cytokines,which may be associated with the pathogenesis of silicosis,the detection of these indicators may have significance of diagnosis,staging,disease monitoring and prognosis of the diseases.

ABSTRACT

Objective To describe the expressions of programmed death-1 (PD-1) and its ligand PD-L1 on the surface of peripheral blood lymphocytes in patients with tuberculosis.Methods A total of 77 cases of pulmonary tuberculosis were recruited,of which 27 were single infection,41 were coincident with bacterial or fungal infection and 9 patients with diabetes mellitus.Twenty-nine healthy donors were also enrolled as control group.The expressions of PD-1/PD-L1 on the peripheral blood lymphocytes and mononuclear cells were detected using immunostaining and flow cytometry.Collected data were analyzed with t-test statistics.Results Among the three groups of tuberculosis including pulmonary tuberculosis,pulmonary tuberculosis coincident with infection and pulmonary tuberculosis with diabetes mellitus,the percentages of CD4+ CD25+ T cells as well as CD4+ CD25high T cell subsets were both significantly higher than those in healthy controls (t=4.892,4.635,4.974,5.407,4.660,5.279,all P<0.01).The expression of PD-1 was up regulated on the surface of CD8+ T cells in the groups of tuberculosis as compared with the control group (t=6.392,8.249,7.072,all P<0.01).The proportions of PD-L1 expressed on the monocytes in each group were (42.51 ± 7.54) %,(49.42± 6.29) % and (48.46 ± 14.58)%,respectively.The difference was significant in contrast to the control group,which was (17.91 ±3.03)% (t=5.168,6.854,5.665,all P<0.01).PD-L1 expression was also up-regulated on B cells in each group and much higher than that of the control group (51.51±7.32)%,(50.85±7.09)%,(55.66±14.29)% vs (40.11±4.25)% (t=2.562,2.046,2.766,all P<0.05).Conclusion PD-1/ PD-L1 co-inhibitory pathway could be closely related to the development of tuberculosis and its complications,which is involved in the attenuation of anti-tuberculosis and anti infection immune responses.

ABSTRACT

Objective To study the species and drug resistance of Mycobacterium isolated from patients with spu-tum smear positive for acid-fast bacillus in Wuj iang city,and provide reference for the prevention and control of tu-berculosis. Methods Sputum specimens with positive smear were cultured,isolated bacteria were identified and performed drug susceptibility testing,drug resistance among different species of strains and between patients with initial and repeated treatment were compared.Results A total of 1 03 Mycobacterium isolates were included in the study,13 of which were nontuberculous Mycobacterium,drug resistance rate was 100.00% ,multidrug resistance (MDR)rate was 84.62% ;90 isolates were Mycobacteriumtuberculosis,81(90.00% )of which were Mycobacteri-umhominis. Drug resistance rate of Mycobacteriumtuberculosiswas 35.56% ,MDR rate was 14.44% . Of 70 ini-tially treated tuberculosis patients with positive sputum smear,14(20.00% )were resistant to drugs,MDR rate was 4.28% (3/70);Of 20 repeatedly treated tuberculosis patients with positive sputum smear,18(90.00% )were resist-ant to drugs,MDR rate was 50.00% (10/20).Conclusion Mycobacteriumtuberculosisis the major isolated strain from patients with positive sputum smear. Drug resistance and MDR rates of nontuberculous Mycobacterium are very high. Drug resistance and MDR rates of Mycobacteriumtuberculosisin repeatedly treated patients are higher than initially treated patients.

ABSTRACT

Objective:This study was to analyze the changes of CD4~+ T lymphocytes and their subtypes,Th1 and Th2 cells,in the peripheral blood of patients with pulmonary tuberculosis disease and to investigate their clinical significance in the pathologic process of pulmonary tuberculosis.Methods:For polarization measurement of T-helper cells,1∶100 diluted Ionomycin and 1∶10 diluted Monensin were added in sequence into the equivalent volume mixture of heparin anti-coagulated whole blood and RPMI-1640 culture liquid.After being well mixed,the mixture was incubated at 5% CO_2,37℃ for 4 hours or overnight.To 100 μl of the mixture and in sequence,the antibodies of CD3-PerCP,CD8-APC,mIgG1-FITC,Rat IgG1-PE,IL-4-PE or IFN-γ-FITC were added.The stained CD4~+ IL-4~+ (Th2) and CD4~+ IFN-γ~+ (Th1) were analyzed by flow cytometry.Results:Compared with those from healthy controls,the peripheral blood of pulmonary tuberculosis patients contained significantly fewer Th1 (P<0.01) but significantly more Th2 cells (P<0.05).Th1 cells in the peripheral blood of the patients with miliary pulmonary tuberculosis were obviously fewer (P<0.05) than in infiltrative pulmonary tuberculosis patients.The amount of Th2 cells in the peripheral blood of the patients with miliary pulmonary tuberculosis was significantly more (P<0.05) than in either infiltrative pulmonary tuberculosis or tuberculous pleurisy patients.The ratio of CD4~+/CD3~+ tended to decrease in all these patients,and it was much lower (P<0.05) in the patients of miliary pulmonary tuberculosis than in infiltrative pulmonary tuberculosis patients.Patients suffering from both pulmonary tuberculosis and diabetes had significantly lower levels of Th1 cells and CD4~+/CD3~+ (P<0.05) and more Th2 cells,compared with those of pulmonary tuberculosis patients without diabetes.Levels of Th1 and Th2 cells restored significantly (P<0.05) in 15 severe pulmonary tuberculosis patients after receiving tuberculosis chemotherapy and microcalorie theropy for three months.Patients with positive sputum examination tended to have decreased Th1 and CD4~+/CD3~+ (P>0.05) and significantly increased Th2 (P<0.05).Conclusion:Immunosuppression existed,in different extents,in patients of infiltrative pulmonary tuberculosis,tuberculous pleurisy,miliary pulmonary tuberculosis as well as patients with both pulmonary tuberculosis and diabetes.Analysis of Th1,Th2 and CD4~+/CD3~+ may be benefit for the judgments of disease conditions and therapeutic effects.

ABSTRACT

Objective To observe the effect of treating diabetic peripheral neuropathy(DPN) with methylcobalamin and Chinese medicines with the funetious of nourishing Yin and promoting blood circulation. Methods 72 patients with DPN were randomly recruited into a control group and a treatment group.Symptoms, NCV,value of blood routine examination,blood biochemistry check and ECG were compared before and after the treatment. Results The total effective rate was 88.0% and 68.2% in the treatment group and the control group respectively,showing significant difference (t=3.14, P<0.01).The value of blood routine examination, blood biochemistry check (except blood sugar level) and ECG showed no obvious changes before and after the treatment in both groups.Conclusion Therapy of methylcobalamin Tablets with Chinese medicines of nourishing Yin and promoting blood circulation is effective on DPN and worth of clinical promotion.

ABSTRACT

Objective To study the effects of Tan Ⅱ A on the expression of c-Met and HIF-lα in gastric cancer SGC7901 cells under hypoxia and the correlation between c-Met and HIF-lα expression. Methods The model of hypoxia was established by CoCl2. There were three groups; normal control group, hypoxia control group, and hypoxia combined with different concentrations of Tan ⅡA group. After Tan ⅡA was added to the media with 0.5, 2.0 and 10.0 mg/L respectively for 48 h under hypoxia, the expression of HIF-1α and c-Met protein was detected by immunocytochemical staining. Results Immunocytochemical staining revealed that the expression of HIF-1α and c-Met protein was inhibited by Tan ⅡA in a dose-dependent manner under hypoxia. High positive correlation was found between the expression of HIF-1α and c-Met (n = 4, r = 0. 996, P <0.01). Conclusion Tan ⅡA might inhibit the expression of c-Met protein via down-regulation of HIFlα protein expression under hypoxia, suggesting that Tan ⅡA might play an important role in fighting against hypoxia-stimulated invasion and metastasis of tumor.

ABSTRACT

Objective To observe the effect of treating DPN with xuesaitong tablet and mecobalamine. Methods 112 patients with DPN were randomly recruited into a treatment group (60 cases) and a control group (52 cases). The control group was treated with mecobalamine, 0.5 mg. Rid., while the treatment group was treated with Xuesaitong tablet, 50mg tid.,On the basis of the control group. Both groups were treated for successive three months. Results The total effective rate was 78.33% and 57.70% in the treatment group and the control group respectively, showing a statistical difference (t= 1.9804, P=0.0271). Conclusion The combined therapy of xuesaitong tablet and mecobalamine was more effective on DPN than mecobalamine used only.

@#Objective To provide guidance for reconstructing the sensation of the anterolateral thigh flap (ALTF) used to repair extensive soft tissue defects in heel. Methods Choose 7 adult male corpses, take the nerval samples respectively from the lateral femoral cutaneous nerve (LFCN) 5cm below the anterior superior iliac spine (ASIS) and the initial segment of the medial caleaneal nerve (MCN) and the lateral calcaneal nerve (LCN), fixed, dewatered gradiendy, embedded, located, and made them into semithin sections, dyed with toluidine blue. The pictures were taken by a medicine figure imaging analysis system named MOTICMED 6.0, observe the nerves's sectional morphous, the quantity and distribution of their nerve fiber bundles, count the quantity of nerve fibers and determine the density of them. Use Photoshop 7.0 version precinct software for measuring and calculating the area of the nerve fiber bundles and the Photoshop grid function was used to measure the density of the nerve fibers. Results In our cross-section study, the median number of nerve bunches in LFCN, MCN and LCN1, was 4, 3 and 4, respectively. The median number of nerve fibers' area was 114.8 um2, 126.92 um2 and 102.76um2, respectively. The median number of nerve fibers' density was 11.43/um2, 6.47/um2 and 10.08/um2, respectively. The median number of nerve fibers was 987, 862 and 570, respectively. Conclusion The MCN and the LCN1 are ideal cutaneous nerves to suture with LFCN in the ALTF used to repair widespread soft tissue defects in heel because they have similar histomorphological characteristics with the LFCN.

ABSTRACT

Objective To investigate the effects and values of extracorporeal membrane oxygenation (EC-MO) used in patients after cardiac arrest. Method During five years period from June 2002 to June 2007,fifteen cases taken for cardiopulmonary resuscitation were treated by using ECMO in the emergency department and ICU. All the measures for disgnosis and treatment were observed to the guidelines for cardiopulmonary resuscitation and emergency cardiovasculat care set by the American Heart Association in 2005, and ECMO was applied in addition. The study was a self-comparison trial. The biomarkers including heart rate (HR), mean arterial pressure (MAP), central venous pressure( CVP) , arterial partial oxygen pressure (PaO2), arterial partial pressure of carbon dioxide ( PCQ2), oxygen saturation (SaO2), hydrogen power (PH), and concentration of lactic acid were taken and assayed before and 10 min, 1 h,6 h, 12 h, and 24 h after treatment. Differences between the results of measurements were analysed by t -test for matched pairs using SPSS version 10.0 software package. Neurological sequelae was also observed and described. Results Ten minutes after ECMO treatment, MAP rose dramatically (P < 0.01) and got to further higher level one hour later ( P < 0.05). On the other hand, CVP dropped off 10 minutes after ECMO treatment and further lowered one hour later, and PaO2 and SaO2 were improved apparently ( P < 0.01), and pH was increased significantly (P < 0.01), while a derease in concentration of arterial LA of significantly (P < 0.01). Of the 15 cases, 11 patients got well,including completely recovered without sequelae in 5,memory disorders in 2 and hemipleggia in 2 patients. Conclusions ECMO provides effective blood flow to hearts, and therefore, improves oxygenation and oxygen consumption greatly. Moreover, it provides stable oxygenated blood to brain and therefore, it is a good approach to cardiopulmonary cerebral resuscitation.

ABSTRACT

<p><b>OBJECTIVE</b>To prepare and apply monoclonal antibodies (McAb) against recombinant human interferon alpha (rHu IFN-alpha).</p><p><b>METHODS</b>Five cell lines (2E9, 4G1, 2A7, 2C9, 4G10) secreting McAbs against rHu IFN-alpha were established by hybridoma technique.</p><p><b>RESULTS</b>All the cell lines secreted monoclonal antibodies stably. Functions of secreting antibodies of the five cell lines lasted for 6 months in BALB/c mice and 8 months in cell culture. The specificity of antibody was constant. The Ig subclasses of the McAbs were IgG1. Anti-IFN McAb affinity purification column was prepared by coupling the anti IFN-alpha McAb to Sepharose 4B. The combining rate reached was higher than 95%.</p><p><b>CONCLUSIONS</b>The highest purification efficiency was obtained by using 4G10 column.</p>

ABSTRACT

Objective To investigate the factors which affect the bone union of distracted region after limb lengthening, so as improve the curative effect and diminish the incidence of complication. Methods To look up the latest literatures dealing with the bone union in limb lengthening, then review the procedure of osteogenesis and the affecting factors. Results The osteogenesis of distracted region after limb lengthening is a sophisticated procedure. It can be affected by the velocity of lengthening, the period of lengthening, the site and method of osteotomy, the age etiology of patient. Conclusion The bone union of distracted region after limb lengthening can be facilitated by following factors :① the velocity of lengthening slower than 1.0 mm/day; ② moderate delay in distraction; ③ axial shortening of distracted region; ④ micromovement stimulation.

ABSTRACT

To study the expression of bone morphogetic protein(BMP) and transforming growth factor β1(TGF-β1) in regenerative gap during lengthening period. Method: The right tibia of 14 mature rabbits were lengthened using metaphyseal osteotomy. Distraction was started on the 4th day postoperatively, 1mm per day and conducted twice each day. The expression of endogenesis BMP and TGF-β1 in regeneraive gap were observed during ten weeks after distraction by histology and immunohistochemisty. Results: The result showed that bone morphogetic protein(BMP) and transfoming growth factor β1(TGF-β1) were expressed in chondrocytes, osteoblasts, and fibroblasts. The expression of BMP was at a high level in initial two weeks after distraction while the expression of TGF-β1 was increased step by step. Then both of them decreased alonged with lengthening proceeding. Two weeks after distracting stopped, the expression of BMP could not being observed. But the expression of TGF-β1 could being observed all stage after distraction stopped. Conclusion: BMP and TGF-β1 were expressed persistently in regenerative gap during bone lengthening.

ABSTRACT

The operative methods for treatment of thoracolumbar fracture-dislocations have been improved in recent years. Anterior or anterior-lateral decompression for thoracolumbar fracture-dislocations with spinal cord injury is considered as a practical and successful method by many researchers. Since 1994 using trans-pedicular partial vertebrectomy, we have treated 43 cases of late thoracolumbar fracture-dislocations with paraplegia successfully.

ABSTRACT

A new method to reconstruct a thumb was worked out in the past few years. It was achieved by inverting the skin flap of the amputated thumb stump and suturing it to an island-shaped skin flap with nerves and vessels from the dorsum of the index finger. A piece of artificial bone was inserted between the two skin flaps into the first phalanx. The operative procedure was described in detail and the indications, advantages and disadvantages of this method were discussed. It is suggested that this method be used to reconstruct a thumb since it is simple and effective.
